Patients are seen with in 24-48 hrs after a referral is received

To qualify for this care, you must:
*Be under a doctor's care. The doctor must order home care.
*Be unable to leave home because of illness or injury.
*The exception is that if you do leave home it is for short periods of time and it requires considerable effort, supportive devices, or the assistance of another person.
*Need skilled health care services on a part-time, intermittent basis.
*Have a need for Nursing, Physical Therapy, Occupational Therapy or Speech Therapy.
