What You'll Do:

•Perform home health nursing visits for a diverse patient population,
•Complete comprehensive assessments and documentation using OASIS.
•Develop, coordinate, and implement individualized patient care plans.
•Communicate regularly with physicians, therapists, and family members.
•Manage your caseload efficiently and independently while maintaining high-quality
clinical care.


What We're Looking For:

•Licensed Registered Nurse (RN) in Michigan,
•Strong clinical skills with a solid understanding of home health protocols.
•Previous experience with OASIS documentation is required.
•Excellent organizational, time management, and communication skills.
•Ability to work independently while remaining highly collaborative with the care team,
